You have a number of things that cause this. The most common is the drive box that has a fibre gear inside which wears. This is situated on the front of the engine, near the fan. If worn it has to be replaced. next most common is a broken drive cable. this usually only happens if the cable is routed wrongly and has a sharp bend, and then of course there is the tacho unit itself,which, unless you are an instrument repairer is not usually simple to fix.
